Lymphogranulomatosis (venereal)
Venereal lymphogranulomatosis is a venereal disease. Many people know an oncological disease called lymphogranulomatosis, which proceeds malignantly with an increase in lymph nodes. But few people know venereal lymphogranulomatosis, which is sexually transmitted. The disease is also characterized by inflammation of the lymph nodes. Unlike malignant lymphogranulomatosis, early treatment with antibiotics has a favorable outcome. Venereal lymphogranulomatosis is caused by the bacteria Chlamidia trachomatis. There are 3 known biovars of this pathogen. The first biovar causing trachoma includes serovars A—C; the second biovar causing urogenital chlamydia includes serovars D—K; the third biovar causing venereal lymphogranuloma (LGv-biovar) includes serovars L1—L3. Serovars of venereal lymphogranulomatosis have a tropicity to lymphoid tissue and affects inguinal, femoral and deep pelvic lymph nodes. The disease is not typical for our region. In Russia, the disease is extremely rare. These are mainly imported cases of the disease, from countries with a tropical climate.
This ailment occurs during promiscuous sexual intercourse with patients. In rare cases, it is transmitted by household means through personal hygiene items and medical manipulations.
symptoms
Venereal lymphogranulomatosis begins with the appearance of small bubbles on the skin, which quickly open and turn into erosions or ulcers. Rashes mainly appear on the external genitalia. Representatives of the stronger sex may have rashes in the form of a chancre on the penis. And in women on the mucous membrane of the genitals. If the infection got oral or anal sexually, then rashes may appear on the lips, oral mucosa, or in the anal area and in the rectum. Ulceration heals in a few days. From the moment of infection to the appearance of rashes, approximately 7-10 days pass. The incubation period is from 5 to 21 days. Before the appearance of a rash on the skin, there may be weakness, poor health, headaches, an increase in temperature.
After 1-4 weeks, the pathogen spreads lymphogenically to the lymph nodes, mainly in the inguinal. Hence the other name of the disease – inguinal lymphogranuloma. The affected lymph nodes increase in size, become dense, painful. The skin above the affected nodes acquires a bluish-red color. In women, the lymph nodes of the small pelvis are most affected with the involvement of the rectum in the process. If the inguinal nodes are not treated, then suppuration of the lymph nodes and the formation of fistulas, strictures, deformities occur.
diagnostics
With the advent of accurate diagnostic methods, it is not difficult to establish a diagnosis. In the presence of a characteristic clinic of lymphogranulomatosis, it is necessary to exclude the imported case. If after a rest or a business trip in dangerous places due to venereal lymphogranulomatosis, an ulcer or a bubble appeared on the genitals, this gives reason to suspect the disease. Laboratory examination includes:
seeding of material from lymph nodes;
serological tests (ELISA, PCR, RIF).
treatment
Early forms of venereal lymphogranulomatosis are treated with antibiotics very successfully. Treatment of late forms is reduced to surgical treatment. It is necessary to start treatment at the first signs of the disease. With late forms, irreversible processes begin, which are practically not treated and directly complicates the life of people.
