Today, dermatologists find it difficult to give the most accurate explanation of the reasons for the manifestation of this complex and aggravated form of acne in humans. In general, several common types and mechanisms of the development of this disease are assumed. Among them are infectious—allergic and toxic-allergic mechanisms. Today it is known for certain that lightning acne manifests itself on the human body against the background of certain diseases of an inflammatory nature, namely:
ulcerative colitis;
Crohn’s disease;
hepatitis;
chronic gastritis;
pyelonephritis;
chronic bronchitis.
There are cases when many patients who have become ill with lightning acne, note the manifestation of the first signs after taking some antibiotics from the tetracycline series, as well as in connection with treatment with isotretinoin or the use of diagnostic procedures using multiple synthetic retinoids.
Symptoms of lightning acne
In most cases, lightning-fast acne occurs against the background of the current mild form of the disease with papulo-pustular or nodular cystic types of acne. It should be noted that lightning acne is characterized by such common symptoms as an unexpected onset of the disease in combination with ulcerative necrotic rashes, as well as with a sharp deterioration in the general condition of a person.
The main sign of lightning acne is a rash localized on the skin in the following places:
in the chest area;
in the shoulder area;
on the side of the neck;
on the back.
In appearance, lightning-fast acne most resembles a large accumulation of acne, presented in the form of papules or nodules. In most cases, these elements are located only on a hyperemic background, as a result of which they are combined with pustular rashes. Due to this clinical picture, many dermatologists are of the opinion that this type of disease is most similar to gangrenous pyoderma.
The main symptoms of lightning acne are the following:
deterioration of the general state of health;
sudden temperature rises;
severe pain in the muscles and abdomen;
anorexia;
arthralgia.
Diagnosis of lightning acne
The main indicators in the diagnosis of lightning acne are the manifestation of rash elements, a sharp and very rapid onset of the disease, as well as significant changes in the general well-being of the patient.
If we talk about the blood test, the study usually records accelerated ESR, a decrease in hemoglobin and, of course, leukocytosis.
Bacterial culture of blood gives a negative result when checking it for sterility. In general, back-sowing is initially carried out in order to determine the sensitivity of pathogenic microflora, but in a particular case, only what is separated from the surface of the infected elements is checked.
In order to identify and cure lightning acne and other concomitant chronic pathologies, the following consultations are prescribed to the patient:
consultation of a pulmonologist;
consultation of a qualified gastroenterologist;
consultation with a qualified urologist or gynecologist.
It is also quite possible to conduct additional research, such as:
gastroscopy;
irrigoscopy;
lung radiography;
colonoscopy;
Ultrasound of internal organs (bladder, kidneys);
bronchography;
urography.
If it so happened that lightning acne appeared in women, then they will additionally need to consult not only a gynecologist, but also an endocrinologist to resolve the issue of using therapy with antiandrogens or estragens.
Treatment of lightning acne
One of the main elements of general therapy for lightning acne is the appointment of the following antibiotics:
doxycycline;
clindamycin;
erythromycin.
Some medications that prevent inflammatory processes are also used:
ibuprofen;
nimesulide;
nurofen and others.
If we talk about local treatment, it is accompanied by the use of special ointments and gels. The composition of the rubs includes an antibacterial component. In addition, such types of physical therapies as ozone therapy, phototherapy, UFO and mesotherapy are used.
